MEDICAL RELEASE FORM
We, the staff of Faith Over Fear Tumble & Cheer recognize our obligation to keep our
students and their parents aware of the risks and hazards associated with the activities of
tumbling, cheerleading and associated strength training. Students may suffer injuries, possibly
minor, serious or catastrophic in nature. Tumbling, cheerleading and strength training can be
dangerous and can lead to injury. Parents should make their children aware of the possibility of
injury and encourage their children to follow all the safety rules and the coaches’ instructions.
Faith Over Fear staff will only warn the child through “Safety Measures”, our teaching styles,
and progression as instructed by our organization's industry governing bodies. Faith Over Fear
Tumble & Cheer, its coaches and other staff members will not accept responsibility for injuries
sustained by students during the course of tumbling class instruction, open gym times, and
private instruction, or in the course of any exhibition, competition or clinic in which he or she
may participate in the programs offered or partnered with by Faith Over Fear Tumble & Cheer.
WAIVER OF LIABILITY & ASSUMPTION OF RISK
I recognize that severe injuries, including permanent paralysis or death can occur in
sports or activities involving height or motion, those activities including but not limited to
gymnastics, tumbling, trampoline, and cheerleading. I am also aware that participation in day
camps that involves transportation could cause injury or death in a vehicular accident. Being
fully aware of these dangers, I hereby give consent for my child(ren) to participate in any and all
Faith Over Fear Tumble & Cheer programs and activities and I ACCEPT ALL RISKS associated
with this participation. In consideration for my or my child(ren)’s participation I hereby, for myself
and my child(ren) and our respective heirs and successors, PROMISE NOT TO SUE and
FOREVER RELEASE Faith Over Fear Tumble & Cheer, its officers, directors, shareholders,
employees, contractors and volunteers from all liability resulting in damages or injuries incurred
as a result of participation including those resulting from acts of negligence.
MEDICAL AUTHORIZATION
I fully understand that the Faith Over Fear Tumble & Cheer staff members are not
physicians or medical practitioners of any kind. With the above in mind, I hereby release the
staff to render temporary first aid to my child or children in the event of any injury or illness and if
deemed necessary by the staff to call our doctor and seek medical help, including transportation
by a staff members and/or it’s representatives, whether paid or volunteer, to any healthcare
facility or hospital, or the calling of an ambulance for said child should the staff deem this
necessary. I understand that it is the parents’ responsibility to warn the child about the
dangers of tumbling and injury. I, my executors or other representatives, waive and release all
rights and claims for damages that I or my child or children may have against Faith Over Fear
Tumble & Cheer and/or its representatives whether paid or volunteer. I have further read and
fully understand the information above and sign this waiver and release of liability
and am fully aware of its contents and meaning.

POLICIES, TERMS AND CONDITIONS AGREEMENT
COMMUNICATIONS POLICY
Faith over Fear Tumble & Cheer expect all of our students and their families, our coaches and
staff to treat each other with the utmost respect at all times.Our Staff adhere to an open door
communication policy to help each parent and athlete understand every decision in which they
are involved, but do ask that it is communicated in an appropriate and respectful manner. We
strongly encourage that parents have their athlete speak with coaches directly in order to
remove any feelings of a communication barrier. All communications via email and/or text
message from under age athletes to staff members must also include the parent as a recipient.
Announcements concerning class times, cancellations, gym closings and other necessary
information will be communicated to parents via our website, our private group text message
platforms and our social media accounts. During classes and practices, coaches are needed to
instruct the athletes. We ask that parents do not approach a coach during a class or practice.
Any issues may be discussed at the end of the session.If a student and/or members of their
families attitudes or actions interfere with a practice or performance, they will be disciplined,
and, if necessary, removed from the facility or event. If a problem arises with a Faith Over Fear
Tumble and Cheer policy, a staff member, another student or their family member, we ask that
you inform our Director privately and the issue will be addressed discreetly and impartially.
Please keep in mind that while coaches are there to speak openly with you about any questions
or concerns regarding your athlete, we will never speak about a decision made regarding
another athlete unless they and/or their parent is present to give consent to share information.
FACILITY POLICY
Only Member Families and director approved visitors are allowed on the gym premises and
must check in at the Front Desk/Office. You must come inside the gym to sign in and out your
student(s). No parking lot drop offs or pickups are permitted unless prior clearance with
the front desk to ensure the children are escorted to and from their vehicle by a staff member.
Students will only be released to their listed parents or designated pick-up person indicated on
their registration form. Unattended children are never allowed on the gym premises and parking
areas. Loitering in the adjacent field, businesses and homes is strictly prohibited. These areas
are private property and trespassing is illegal, punishable by law. Due to space inside the facility
and insurance restrictions, Parents and Visitors must remain in the designated waiting/viewing
areas at all times and be respectful and quiet. Only Faith over Fear Staff and students enrolled
in that time slots class are allowed on the mats/practice areas. Students are only allowed in the
mats/practice area during their enrolled class or lesson. Parents and approved visitors are not
allowed in the mats/practice areas or use of equipment at any time, except during designated
Parent classes.
ATTENDANCE
MAKE UP CLASSES: We understand that there will be times when your student can not make
their class or private lesson that has already been paid in advance. To ensure our families get
the most of their investment, we will offer Make-up Classes for those students that have prior
notice absences. (A Minimum notice of 30 minutes before the start of class is required for all
absences). A Make-up Class Day for certain classes with a large amount of absences will be
scheduled once a month of Saturdays. You can register for your make-up class by calling or at
the front desk. Due to class size restrictions, make-up classes may not be available during our
scheduled week nights classes.
HOLIDAY AND REGULAR SCHEDULED CLOSINGS: We do not make up for Holiday or Scheduled
closings. We do not charge extra for 5 week months, therefore we do not make up or pro rates
for Holiday or regular scheduled closings.
INCLEMENT WEATHER CLOSINGS: When Poyen and Sheridan Public Schools are closed due to
weather, will be closed also. If there is a threat of dangerous weather we may close at the last
minute for the safety of our students and staff. We will do everything in our power to notify you
in this case by email, our website, our group text message platform, and social media pages.

MEDIA CONSENT AGREEMENT
I am aware that individual and group publicity photos and videos are taken from time to time and
in consideration for my or my child(ren)’s participation I hereby grant my permission for my
child(ren)’s likeness to be used in Faith Over Fear Tumble & Cheer publicity and advertising.
PAYMENT POLICY
ANNUAL FAMILY REGISTRATION FEE: A fee of $25.00 per family is due at time of
registration. The fee will then be due on that date each year. TUITION: Monthly tuition is due on
the 1st of each month for recurring classes. Special classes, camps and clinics must be paid
before or at time of service. Payments done through automatic back or credit card draft, cash or
check and require a 30 Day written notice on all changes or cancellations. There is a special
weekly payment arrangement that requires approval by management.
DEPOSITS TO HOLD CLASSES: A deposit will be required to hold spots in class if paying by the
week (by special arrangement with Management). You will still be responsible for the entire term
of payment, either monthly or quarterly as indicated at the time of booking.
PAY IN ADVANCE/DISCOUNTS: You may pay in advance up to three (3) months. Discounts may
be available to families with multiple children enrolled and multiple class enrollments. Please
contact us for more details on these packages.
NOTE: WE DO NOT OFFER REFUNDS/ $15.00 on all returned NSF Auto Drafts /$30.00 on all
NSF Checks
If at any time a student owes the gym more than one month’s fee, he or she will be moved to an
inactive roster until the balance is zero. Any student who falls behind in payments may be
removed from classes and all late fees will be charged. Failure to pay will result in Collection
Manners by third party vendors or may result in Small Claims Court Indictment.
PAYMENT POLICY TERMS AGREEMENT
This agreement describes a monthly payment contract for Faith Over Fear Tumble & Cheer
classes and sets forth the terms and conditions between the above named parent and Faith
Over Fear Tumble & Cheer. The terms and conditions are as follows:
PAYMENT
Payment may be cash, check, credit or debit card on the date of your initial enrollment.
Subsequently Faith Over Fear Tumble & Cheer will charge the listed checking account,
credit card or debit card each month on the monthly charge date or the next business day
unless the cancellation form is turned in 30 days prior to the charge.
MINIMUM COMMITMENT
Each subsequent month Faith Over Fear Tumble & Cheer will charge the listed checking
account, credit card or debit card and extend the child’s enrollment exactly one month
from the charge date (1st of each month), until this authorization is cancelled by you,
in writing 30 days prior to the next charge date.
CANCELLATION TERMS
Faith Over Fear Tumble & Cheer will charge the listed checking account, credit card or debit
card and extend the child’s enrollment exactly one month until the authorization is cancelled in
person and in writing. Retroactive cancellation is not possible under this agreement. The
undersigned agrees to inform Faith Over Fear Tumble & Cheer of any change in the checking
account, credit or debit card number, and issuer or expiration date. Should the account be
rejected for any reason, this agreement will immediately lapse and the enrollment will be
discontinued until payment is made. The signer below assumes liability for this agreement, as
stated herein. Thirty (30) days notice is required for cancellation and make-up classes must be
taken within this 30 day period.
I have been made aware, fully understand and agree to follow all of the
policies and procedures of Faith Over Fear Tumble & Cheer, including but not
limited to, attendance, payment responsibility, unattended children, and
facility rules and restrictions.
